Important information about our office policies, financial procedures, and patient privacy. Please review these policies to ensure a smooth experience at our practice.

Financial Policy

Payment & Insurance

  • Payment is due at the time of service unless other arrangements have been made in advance
  • We accept cash, checks, and major credit cards (Visa, MasterCard, American Express, Discover)
  • We participate with most major insurance plans. Please verify your coverage before your appointment
  • Co-payments, co-insurance, and deductibles are due at the time of service
  • If your insurance requires a referral, it is your responsibility to obtain one prior to your visit

Insurance Claims

  • We will file claims with your insurance company as a courtesy to you
  • You are responsible for any balance not covered by your insurance
  • If your insurance does not pay within 60 days, the balance becomes your responsibility
  • Please bring your current insurance card to every appointment

Outstanding Balances

  • Accounts not paid within 90 days may be subject to collection procedures
  • A $35 fee will be charged for all returned checks
  • Payment plans may be available for large balances - please speak with our billing department

Office Policy

Appointments

  • We request 24-hour notice for appointment cancellations
  • Missed appointments or cancellations without proper notice may result in a $50 no-show fee
  • Please arrive 15 minutes early for your first appointment to complete necessary paperwork
  • Please arrive 10 minutes early for follow-up appointments
  • Late arrivals may need to be rescheduled depending on availability

Office Hours

  • Monday - Friday: 9:00 AM - 5:00 PM
  • Closed on weekends and major holidays
  • After-hours urgent care: Please call our main line for instructions
  • For medical emergencies, please call 911 or go to the nearest emergency room

Prescription Refills

  • Please allow 48 hours for prescription refill requests
  • Refill requests should be made during regular business hours
  • Some medications may require an office visit before refills can be authorized
  • Schedule your appointment before your medication runs out to avoid delays

Medical Records

  • Medical record requests must be made in writing
  • Please allow 7-10 business days for processing
  • A fee may apply for copies of medical records per state regulations
  • Records will only be released with proper written authorization
